Online Patient Pre-Admission

The Ambulatory Surgery Center (ASC) at Medical Center Clinic offers patients the convenience and privacy of a secure, online registration process. If you are a new patient to our center, please go online today to complete your registration using the personalized link sent to the email provided to your physician’s office. If a link was not received or your email was not provided to us, please call 850.969.2121.

PLEASE COMPLETE 48 HOURS PRIOR TO YOUR SURGERY

You will be asked about your health history, medications, and previous surgeries. Returning patients that submitted a questionnaire within the last 90 days do not have to resubmit their form. Any changes will be addressed when an ASC staff member calls 24 hours prior to your surgery date. It’s important to complete your online registration as soon as possible so that your medical team will have time to review your information prior to your visit. INSTRUCTIONS FOR ACCESSING YOUR PORTAL:

1. A personalized link to access the portal will be sent to the email provided to Medical Center Clinic. If a link was not received or your email was not provided to us, please call 850.969.2121.
2. Click Access Patient Portal at the bottom of the email.
3. Enter your first name, last name, and date of birth.
4. Complete the Pre-Admission Questionnaire and review the Pre-Admin Instructions.
5. Once both areas are complete, close the webpage.
